Check the box for "I agree to pay future charges, when I authorize them, according to card issuer agreement."
Click "Submit for Processing"

As a convenience, you may pre-register a credit card on the submission page to charge your purchases at event Dream Gala 2017.

I authorize JDRF - Northern New England Chapter - 4366 to use the credit card entered on the submission page at event Dream Gala 2017. Once the transaction has completed successfully, a confirmation receipt will be sent to the email address entered.

T1D is an autoimmune disease in which a person's pancreas loses the ability to produce insulin — a hormone essential to turning food into energy. It strikes both children and adults suddenly and is unrelated to diet and lifestyle. It requires constant carbohydrate counting, blood-glucose testing, and lifelong dependence on injected insulin.
